Level Select

Press [Right][Up][Down], [Right][Up][Down], [Right][Up][Down], [Right][Right][Right] on the main Star Wars logo/story crawler. The music changes to confirm entry. When you die, the continue screen has a level select on it (use [Left][Right] to select a level).
